Community Care Physician Network, UnitedHealthcare Partner To Serve Medicaid Beneficiaries In North Carolina
Community Care Physician Network (CCPN) and UnitedHealthcare recently established a new Advanced Care Medical Home and care management network to serve Medicaid beneficiaries in North Carolina. Specifically, the collaboration will give Medicaid beneficiaries access to CCPN’s 2,200 primary care providers throughout the state when the new managed care system launches in 2019.
By bringing CCPN’s primary care practices to UnitedHealthcare’s Medicaid beneficiaries, the two organizations anticipate being able to enhance care quality and experience while lowering health care costs.
