Medicare Skilled Nursing Facility Value-Based Purchasing Program (SNF VBP) Exchange Function Analysis

This document was released in May 2018 by the Centers for Medicare & Medicaid Services (CMS). It presents the methodology that Medicare is using for the Skilled Nursing Facility Value-Based Purchasing (SNF-VBP) program. The SNF-VBP evaluates SNF all-cause 30-day hospital readmission rates during the performance year compared to the benchmark year. SNFs that have lower rates than the national benchmark earn incentive payments, an increase to the prospective payment system (PPS) rate.
