August 13, 2010 The Oregon Department of Health and Human Services contracted with Liberty Healthcare Corporation to conduct a quality and compliance review on Oregon State Hospital (OSH). On August 13, 2010, Liberty's review team released a draft report which noted  that a key problem had been lack of adequate planning and coordination of the improvement efforts. The review team recommended that the OSH leadership prioritize and organize the planned changes to establish clear authority and accountability for accomplishing the changes.
