Connecticut Person-Centered Medical Home Plus Program Wave 3 (PCMH+W3) Request For Proposals, Addendum 1 Questions & Answers & Transcript Of Bidders Conference

On October 2, 2019, the Connecticut Department of Social Services released this addendum to the third wave of the Person-centered Medical Home Plus Program (PCMH+) request for proposals (RFP). The addendum revises sections of the proposal requirements and includes; clarifications to the question and answers portion of the RFP; the date of the Wave 3 bidders conference; and an addendum acknowledgment sheet to be signed and returned by respondents.
