March 23, 2011 Medicare Advantage Special Needs Plans (SNPS) serving people eligible for both Medicare and Medicaid—dual eligibles—often fail to adequately describe their care coordination models such that it is not possible to evaluate whether the SNP provides good care coordination. The SNPs are required to submit their plans to the Centers for Medicare and Medicaid Services (CMS), but a review by the Medicare Payment Advisory Commission (MedPAC) found that the models lacked sufficient description of care coordination activities.
