July 14, 2011 San Diego County released the request for proposals for transition services in July 2011, with proposals due on September 7, 2011. The county sought a contractor to design and implement a project that integrates coaching, mentoring, and teaching strategies resulting in a successful transition to independent living and an increase in the number of transition age youth and young adults between the ages of 14 and 25 transitioning from the countyÂ’s child or adult mental health systems of care.
