July 25, 2011 On July 15, 2011, the Illinois Department of Healthcare and Family Services released the care coordination plan and timeline for rollout. Care coordination plan is a part of statewide extension of mandatory integrated care program (ICP) for disabled adults started by Illinois Medicaid.
