Ensuring That Patient Centered Medical Homes Effectively Serve Patients With Complex Needs
October 1, 2011 In October 2011, the Agency for Agency for Healthcare Research and Quality (AHRQ) released a Patient-Centered Medical Home Decisionmaker Brief that identified strategies to address barriers that primary care practices face in trying to provide high-quality, comprehensive, coordinated, and accessible care to patients with complex needs. The three strategies are: 1. Reforming primary care practice payment policy; 2. Supporting programs that collaborate with primary care practices to enhance their clinical capacity and offering care managers and other resources to help patients and their families coordinate service; and 3.
