Housing Smart is a partnership between Resources for Human Development, Inc. (RHD), Temple University Hospital’s Center for Population Health, Keystone First, and the Health Partners Plans. The program is designed to improve the health outcomes of people experiencing homelessness by providing them with subsidized housing and community based supportive services.
