U.S. Department of Health and Human Services, Centers for Disease Control and Prevention announces the availability of funding for implementation of the Racial and Ethnic Approaches to Community Health Grant. The purpose of this grant is to create healthier communities by strengthening existing capacity to implement locally tailored evidence- and practice-based population-based improvement strategies in priority populations experiencing chronic disease disparities and associated risk factors, and supporting implementation, evaluation, and dissemination of these strategies.
