California's Santa Cruz County Human Services Department seeks a contractor to provide eligible clients with housing support services. Services performed under this agreement shall implement evidence-based Housing First and Rapid Re-Housing practices with all participating clients. These services shall include outreach to engage at-risk clients, housing navigation that recruits landlords willing to rent to participating clients; identification of appropriate housing opportunities for eligible clients; case management; advocacy; and housing assistance, including referrals for temporary rent subsidies and other financial assistance . . .
