Executive Summary

Created under the Patient Protection and Affordable Care Act (PPACA) of 2010, Health Homes are a population-based integrated care service delivery model for individuals with chronic conditions and serious mental illness (SMI). Health Homes are a Medicaid quality initiative aimed at controlling health care costs by enabling individuals with chronic care illnesses to better manage their conditions, reduce the need for hospitalizations, and avoid costly emergency department visits. Since the implementation of Health Homes, enrollment has fluctuated. This report explores trends in Health Home enrollment, reimbursement, payment rates and implementation.
