Oregon-based Advanced Health, which is focused on coordinating care in Oregon’s Coos and Curry counties, is partnering with social care needs software company Unite Us to address health disparities due to health-related social needs. ;The Unite Us Connect Oregon network offers a central point of contact, through which health and social services provider organizations can securely access and refer people to needed services while monitoring progress and measuring outcomes.
