Affiliated Network Providers & Aurora Behavioral Health Partner For Services, Care Collaboration In Arizona 

Arizona-based Affiliated Network Providers (ANP), Aurora Behavioral Health, and Alera Health recently entered a high value collaborator (HVC) agreement. The partnership aims to improve care coordination between Aurora’s inpatient care and ANP’s outpatient health homes.  

The agreement represents a commitment by both organizations to integrate resources, expand data sharing, and coordinate whole-person care. ANP and Aurora will exchange electronic referrals through the Alera Health Care Optimization Suite, which includes care navigation and alerts.;In addition, they will work together from a consumer’s initial level of care assessment to evaluate the most appropriate clinical disposition . . .
