Aledade & Azara Healthcare Partner To Support Community Health Centers   

Primary care provider network Aledade is partnering with population health and value-based care solution provider Azara Healthcare. The partnership will benefit community health centers that are mutual clients of both companies.  

Clients of both Aledade and Azara will be able to combine Aledade’s playbook, which includes a proprietary technology platform and in-person coaching, with actionable data and analytics provided by Azara DRVS platform.
