Blue Cross Blue Shield Of Arizona Partners With Monogram Health To Improve Chronic Kidney Disease Outcomes

Blue Cross Blue Shield of Arizona (AZ Blue) partnered with Monogram Health to provide in-home care and benefit management services for AZ Blue members with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). AZ Blue will use Monogram’s network of in-home care professionals and support services to improve the care of their members.
