Blue Cross North Carolina, Landmark To Deliver Home-Based Medical Care To Consumers With Complex Chronic Conditions

Blue Cross and Blue Shield of North Carolina (Blue Cross NC) recently announced it is partnering with Landmark to deliver home-based medical care to consumers with complex chronic conditions. These conditions include health ailments such as diabetes, COPD, and congestive heart failure.

The partnership will begin providing this home-based medical care to Medicare Advantage consumers beginning in June 2018. It is expected that about 6,500 consumers throughout the Triangle, Triad, Charlotte, and Fayetteville regions of North Carolina will be benefitted.
