CarePlus Health Plans, a Humana unit operating;in Florida’s Medicare market, is now offering a community navigator platform. ;The tool was designed to address social determinants of health. ;

The tool helps users find and connect with local resources, including;food banks, transportation, and affordable housing options. ;According to CarePlus, initial use shows almost half of searches were related to food security. ;

CarePlus said the navigator is part of a larger effort that involves its network of providers and community partners to use local health data insights to better serve local Florida;communities . . .
