Three new states—Illinois, Kansas, and New Hampshire—were awarded planning grants to implement the federal Medicaid Money Follows the Person (MFP) program to help people with disabilities transition from institutional care to their homes and communities. The program provides an enhanced Medicaid match rate for transition planning and for home-and community-based services (HCBS) provided to support people after they transition from an institution to living in the community.
