Collaborative Bridges Delivering A New Model Of Care To Close Gaps In Behavioral & Mental Health Services On West Side Of Chicago

Three safety net hospitals and three community mental health provider organizations serving the West side of Chicago, Illinois partnered to establish Collaborative Bridges. The program's goal is to ensure that consumers discharged by the hospitals after treatment for mental health or addiction-related conditions are connected to community-based follow-up care.

Collaborative Bridges is part of the Illinois Department of Health and Family Services' Healthcare Transformation Collaboratives (HTC). HTC establishes collaboratives of different Medicaid safety net service providers to close gaps.
