Community Care Physicians Signs Agreement With Humana

Humana has signed a value-based agreement with Community Care Physicians (CCP) to provide in-network access for Humana Medicare Advantage members at CCP facilities. CCP is the largest independent multispecialty medical group in the Capital Region of New York.

Members gaining access to the CCP facilities will include those under Humana’s HMO, PPO, and PFFS Medicare Advantage plans.
