CVS Health Foundation Awards $2.6 Million To Expand Maternal Diabetes Program In New York City
The CVS Health Foundation awarded $2.6 million to the American Diabetes Association to expand a maternal health program focused on gestational diabetes (GDM) in New York City. The expansion adds two additional communities and builds on an initial pilot at Wyckoff Heights Medical Center in Brooklyn.
The program focuses on improving early screening and care coordination for individuals with gestational diabetes.
The expansion includes:
Development of electronic medical record dashboards to support referrals among obstetric, primary care, and endocrinology providers
Increased access to diabetes management tools, including continuous glucose monitoring
