Food Insecurity A Problem For Two-Thirds Of CMS Accountable Health Community Beneficiaries
About 33% of Medicare and Medicaid beneficiaries screened through the Centers for Medicare & Medicaid Services (CMS) Accountable Health Communities (AHC) Model reported at least one core health-related social need. The most commonly reported needs for the first 750,000 people screened were as follows:
67% reported food needs.
47% reported housing stability problems.
41% reported transportation difficulties.
28% reported difficulty paying utility bills.
About 18% of those screened were eligible for community navigation services, and 76% of eligible beneficiaries accepted the navigation assistance. Of the 750,000 people screened, 63% were Medicaid beneficiaries and 37% were Medicare beneficiaries . . .
