Easterseals Florida Expands Disability Health Navigator Program With $250,000 Grant

Easterseals Florida received a $250,000 grant from the Live Well Foundation of South Lake to expand its Disability Health Navigator Program. The two-year funding will support bilingual care coordination and health care access services for individuals with disabilities in South Lake County.

The program is staffed by a team of four bilingual health navigators who provide care coordination and advocacy in community-based settings. Services are tailored to individual needs and include assistance with insurance enrollment, developmental screening for children, immunization education, and navigation of health system barriers.
