Eastside Health Network Announces The Launch Of Four Value-Based Contracts

Eastside Health Network—an integration of Overlake Provider Network and EvergreenHealth—recently announced they have agreed to participate in four value-based care contracts. It is anticipated that the agreements will help to offer about 20,000 consumers access to accountable care programs.

The organizations involved in the partnerships include Regence, Aetna, UW Medicine, and Providence Health Plan. The agreements serve as part of the population health initiative, and seeks to help consumers:

Close gaps in care
Reinforce the appropriate use of hospital emergency rooms
Increase the number of preventive health visits
