Florida’s BayCare Health System Partners With Cadence For Virtual Chronic Care Management

BayCare Health System in Florida partnered with chronic disease management company Cadence Health Solutions to implement a remote consumer monitoring and responsive virtual care program for older adults with chronic health conditions. The new program is intended for BayCare consumers with hypertension, congestive heart failure, and type 2 diabetes. The virtual care program will combine BayCare Medical Group’s expert clinical team with the Cadence platform to provide proactive and preventive health services for adults with chronic conditions.
