HomeCare Connect Introduces Post-Acute Care Connect

To fill the gap in care among injured workers who are discharged from a hospital and need skilled care in the home, HomeCare Connect launched Post-Acute Care Connect (PACC). PACC helps manage injured workers' transitions from hospital settings to skilled nursing facilities by answering a number of questions related to cost and care delivery from workers' compensation carriers and third-party administrators.
