Homeward Health Launches Hybrid, Value-Based Primary & Specialty Care Model For Rural Consumers
After receiving a $20 million investment from General Catalyst’s new Creation Fund, Homeward Health launched operations to provide a hybrid model that uses virtual and in-person services to connect people in rural areas to primary care and specialty services. The company is initially partnering with regional Medicare Advantage plans to provide both in-network primary care services and specialty services, beginning with cardiology.
