Humana & Interwell Health Add Florida To Value-Based Kidney Care Program  

Humana expanded its value-based care agreement with Interwell Health to include Florida. Interwell’s support services are now available to;eligible Humana Medicare Advantage members in Florida with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). ;

Interwell offers comprehensive care and specialized resources, including 2,000 network nephrologists, renal care coordinators, and in-home virtual support from dietitians, nurses, social workers, pharmacists, and care coordinators. The company’s services are now available in 14 states for eligible Humana members with CKD, and in 39 states for eligible Humana members living with ESKD.;
