Humana & Longevity Health Agreement Expands Access To ‘Humana Together In Health’ I-SNP

Humana is partnering with clinical services company Longevity Health. ;Under the collaboration, eligible residents of long-term care facilities in certain states will have access to Humana Together in Health, a Medicare Advantage Institutional Special Needs Plan (I-SNP), beginning in 2025.;

The new agreement covers eligible participants in Indiana, Iowa, Kentucky, Louisiana, New Mexico, Tennessee, Washington, and West Virginia. Humana Together in Health plans include dental, vision and hearing benefits, transportation for eligible medical appointments, and care manager support.;
