I-SNP Longevity Health Enters Multi‑State Value‑Based Joint Venture With Journey Skilled Nursing

Longevity Health, a large Medicare Advantage Institutional Special Needs Plan (I-SNP), launched a multi-state, value-based joint venture with Journey Skilled Nursing. Beginning in June 2026, the two organizations will collaborate to serve SNP members living in Journey’s skilled nursing facilities (SNF) across Georgia, Ohio, and Kentucky.

The partnership marks Longevity's first expansion into Georgia, bringing its total footprint to 16 states and more than 500 SNF communities. The joint venture aims to increase access to coordinated care for residents with complex medical needs while supporting SNF operators through a transparent, value-based model.
