Johnson Memorial Health Partners With Prevounce Health To Deliver Comprehensive Remote Care To Those With Chronic Conditions

Johnson Memorial Health (JMH) in Indiana has partnered with Prevounce Health, which offers remote care management solutions, to launch a combined remote patient monitoring (RPM) and chronic care management (CCM) program that brings personalized, proactive care to patients in any location. The program formally launched in October 2025 and is expected to serve hundreds of patients within the first year, the announcement stated. Prevounce is supporting device fulfillment, patient outreach and education, and more, enabling JMH's clinical staff to focus on escalations and delivering personalized, high-quality care.
