Medical Home Network Brings Expanded Mental Health & Substance Abuse Services To Chicago

A partnership between Medical Home Network and Cook County Health and Hospital System means that member residents will have access to additional mental health providers, substance abuse providers, immediate and urgent care center, and Cermak Health Services (which provides Cook County Department of Corrections health care services).
