Medical Home Network Embeds New Instacart SNAP Screener Into Its Care Management Platform To Address Food Insecurity
Medical Home Network (MHN), which enables federally qualified health centers (FQHCs) nationwide to participate in value-based care, is collaborating with Instacart to help FQHCs address food insecurity. Through the partnership, MHN will embed the new Instacart Supplemental Nutrition Assistance Program (SNAP) eligibility screener into MHNConnect, the company’s care management platform. Instacart launched the SNAP eligibility screening tool on November 21, 2024.
