Medicare Physician Fee Schedule Creates New Advanced Primary Care Management Program

The 2025 Medicare Physician Fee Schedule (PFS) includes provisions for creation of a new preventative care program called Advanced Primary Care Management (APCM), which will incorporate existing care management and telehealth services for ongoing primary care services into a bundle of services. APCM incorporates Principal Care Management, Transitional Care Management, and Chronic Care Management. Unlike existing care management codes, there are no time-based thresholds included in the service elements, which the Centers for Medicare & Medicaid Services (CMS) believes will reduce the administrative burden associated with current coding and billing.
