New York Awards $500 Million To Nine Organizations To Create New Medicaid Social Care Networks
The New York Department of Health (DOH) awarded nine organizations grants totaling $500 million over three years to create new regional Medicaid Social Care Network (SCN) programs. The SCNs are part of the state’s Medicaid Section 1115 Demonstration Waiver. The SCNs will integrate health care and health-related social care needs (HRSNs), such as nutrition, housing supports, transportation, and case management for eligible Medicaid members. The goal is to address health disparities and improve population health.
The nine organizations and their SCN regions are as follows:
