Oak Street Health & Interwell Health Launch Joint Venture, OakWell, To Provide Dialysis Center-Based Primary Care

Oak Street Health (a network of value-based primary care centers for adults on Medicare) and Interwell Health (a kidney care management company) launched OakWell—a joint venture to offer primary care for people with end-stage kidney disease (ESKD) at kidney dialysis centers. OakWell will initially begin offering care services during the third quarter of 2023 at dialysis centers in Chicago, Illinois and Houston and Dallas-Fort Worth, Texas. The goal is to reduce hospitalizations, increase kidney transplantations, and improve outcomes to lower the total cost of care for these individuals.
