Physicians Health Plan Of Northern Indiana Opens Primary Care Center Operated By Premise Health

Physicians Health Plan of Northern Indiana (PHP) opened an exclusive primary care center operated by national primary care company Premise Health. The Premise Health Center in Fort Wayne, Indiana, will care for a select segment of PHP's health plan clients totaling nearly 8,000 eligible members ages two and up.

In addition to comprehensive primary care, the center will offer behavioral health, chronic condition management, biometric screenings, referral management, women's health, and provider-dispensed medications. Care will be provided both in-person and virtually, through phone or video.
