Ripple Health Group Unveils An Enterprise & Consumer Application To Improve Care Coordination

Ripple Health Group (RHG), a health technology company introduced its first two products Care MemoTM and LikePaperⓇ  to help improve care coordination between consumers, caregivers, and health care provider organizations. The goal is to reduce the burden of caregiving by identifying and closing gaps in care. RHG announced that the Care Memo solution is being used by a health insurer that offers Medicare Advantage Institutional Special Needs Plans (I-SNPs) and Dual Special Needs Plans (D-SNPs) to help support he acute consumer population and complex care consumers enrolled in home care.
