Superior HealthPlan Launches New Assessment To Help Texans Find Community Support
Centene’s Superior HealthPlan, which operates in Texas, is now offering a screening tool to help Texans access community resources. The tool is designed to identify and monitor referrals to services considered non-medical drivers of health.
The tool is designed to help connect Superior HealthPlan members with assistance that may impact their overall well-being, such as;transportation, housing, food, education, and other supports. The screener is part of a;health risk assessment that members with complex care needs receive upon becoming a Superior HealthPlan member. The screening is repeated once per year. ;
