Tembo Health & Family Caregiver Alliance Partner For Medicare GUIDE Model Dementia Care In Northern California

Family Caregiver Alliance (FCA) partnered with telemedicine platform Tembo Health to deliver comprehensive, home-based dementia care to fee-for-service Medicare beneficiaries living in Northern California. FCA and Tembo Health are participating in the Centers for Medicare & Medicaid Services (CMS) Guiding an Improved Dementia Experience (GUIDE) Model.

The GUIDE Model services are free for traditional Medicare beneficiaries. FCA and Tembo Health are among the 96 GUIDE Model participants nationwide.
