A fee of $2 per physician name is charged for each request of information. (There is no charge for a physician to seek information on his or her own file.) Hospitals, medical boards, professional and medical societies, malpractice insurers, and certain other health care entities are required to report information to the data bank. Hospitals are also required to check the data bank before hiring physicians or dentists . . .
