Is There A Way To Measure The Success Of The Patient-Centered Medical Home Model?
The Centers for Medicare and Medicaid Services (CMS) has one “medical home” experiment underway, and has just wrapped up two more. The one underway is the Comprehensive Primary Care (CPC) Initiative (which will run over a period of four years). The two that have been completed are the Multi-payer Advanced Primary Care Practice (MAPCP) Demonstration and the FQHC Advanced Primary Care Practice Demonstration.
