Launched in 2012 by ChristianaCare, the Complex Care Community Program is a dynamic initiative that aims to reach a complex consumer population known as “super users of the acute care system.” The goal of the program is to identify individuals who typically only seek care through the Emergency Department (ED) and to connect them with a multidisciplinary team who can provide high quality, coordinated medical, psychological, and social care to reduce unnecessary ED utilization.
