Launched on December 1, 2016, Centerstone Tennessee’s Health Link program provides integrated care coordination and outreach to TennCare recipients who meet mental and physical health criteria. The goal of the program is to leverage a care coordination model that allows for better coordination of behavioral health and physical health services among children, adolescents, and adults with the highest behavioral health needs, as well as to produce improved member outcomes, greater provider accountability and flexibility, and improved cost control for the state.
