Housing Smart is a partnership between Resources for Human Development, Inc. (RHD), Temple University Hospital’s Center for Population Health, Keystone First, and the Health Partners Plans. The program is designed to improve the health outcomes of people experiencing homelessness by providing them with subsidized housing and community based supportive services. The secondary goal of the program is to reduce avoidable health care utilization and link members to primary care and behavioral health support services for this vulnerable population.
