Options Care Management is exactly what its name implies, it is a program for older adults (60-years or older) who need varied degrees of assistance but wish to continue living independently. The goal of the program is to engage with seniors and caregivers to explore options and alternative resources that better fit or augment their financial capacity and caregiving/ self-care capabilities.
