New Jersey Application for a 1915(c) Home & Community-Based Services Waiver

January 1, 2009 This amendment is submitted to consolidate the three Medicaid Waivers currently operated by the New Jersey Department of Health and Senior Services. The Community Care Program for the Elderly and Disabled (CCPED) Medicaid Waiver, Control Number 0032.91.R4 is being amended to include the services in the Enhanced Community Options (ECO) Medicaid Waiver, Control Number 0285.90.R1A and the services in the Assisted Living (AL) Medicaid Waiver, Control Number 0285.90.R1B. Enhanced Community Options and Assisted Living will terminate as of the effective date of the amendment . . .
