In early January 2016, the Illinois Department of Healthcare and Family Services (HFS) notified Medicaid provider organizations about changes to care coordination programs as two program types began transitioning their members to full-risk managed care plans. The transitions affect Care Coordination Entities (CCEs) and Accountable Care Entities (ACEs).
