Transition Planning: A Team Effort
January 1, 1999 In April 1999, the National Information Center for Children and Youth with Disabilities (NICHCY) published a Transition Summary outlining and describing transition options for families with children with disabilities once they graduate high school and public education is no longer guaranteed. Transition services can include instruction, community experiences, the development of employment and other post-school adult living objectives, and (if appropriate) the acquisition of daily living skills and functional vocational assessment.
